Nonprofit hospitals earn their tax exemption by providing community benefit — and the clearest measure is charity care, reported on IRS Form 990 Schedule H. Measured as a share of revenue, Wyoming Medical Center Inc gives back the most among large organizations, devoting 6.7% of revenue to charity care — against a median of 0.9% across 335 reporting organizations (totaling $2.33B in charity care). Large hospitals reporting the smallest charity-care share

Among hospitals with at least $500.0M in revenue that report Schedule H charity care, the lowest charity-care share of revenue. A small share is not the same as zero — these are large, well-resourced systems.

Methodology

Charity care is the cost — not the list-price charges — of free or discounted care a hospital provides to patients who qualify under its financial-assistance policy. Figures come from IRS Form 990 / Schedule H, the annual information return tax-exempt hospitals file. We use each organization's most recent filing with data and express charity care as a share of total revenue so organizations of different sizes can be compared.

The "most relative to size" ranking is limited to organizations with at least $100.0M in revenue; the "least" ranking to those with at least $500.0M. State figures use the median charity-care share across each state's reporting organizations and include only states with at least five. A blank does not necessarily mean zero — some filers do not complete Schedule H. Figures lag by one to two years and reflect the filing organization, which may be a system parent rather than a single facility.
